Purchasing Agreement: Ulta Medical

Ulta Medical was founded to simplify and streamline emergency medical solutions for dental and medical offices. Ulta Medical will track and manage the expiration dates of all of your medications and send you replacement prescriptions automatically. Ulta Medical will work with you directly to customize the right emergency medical kit for your individual practice. Options include automatic medication replenishment, training and support, CE and more.

How to Purchase: To qualify for discount, all orders must be placed directly with Ulta Medical. Contact information for your Ulta Medical rep will be emailed to you once your Agreement has been processed. Your Co-Op Area Director has this information and is available on Member Services.

Discounts and Terms: Discount levels are based on previous year total Cooperative purchasing volume.  Current discount pricing and offers are available on Member Services.

    Once submitted, the Agreement will be sent to our the associated vendor partner for a rep assignment and flagging in their system. Confirmation will be emailed to you when your vendor rep has been assigned.
